Create and Edit Forms

Instead of sending letters to a person A parent, legal guardian, carer, or someone responsible for a student. to complete, sign, and return, you can create and send an online form. Forms can include information and questions to be completed. For more information, go to Forms Overview.

  1. Select Parent Reach Module for IRIS Reach used to send MyEd app messages, texts, or emails to parents and carers for individual or all students. > Form Manager.
  2. The Parent Reach - Form Manager page is displayed. You can add new or manage existing forms.
  1. From the Add New Form panel, enter a Form Name, e.g. Zoo Trip, then select Go.

    To edit an existing form, from the Manage Active Forms panel, select the form Name.

  2. The Parent Reach - Form Manager - Edit Form Step 1 page is displayed. The Form Name is populated with the name you entered. If required, you can edit the Form Name.
  3. The Start Date is populated with today's date. Select or enter the date you want the form to be available to be sent.
  4. The Stop Date is populated with today's date. Select or enter the date on which responses for the form can no longer be submitted.
  5. Enter information you want to send to recipients about the form in Header Contact Info, for example, Complete all options before submitting.
  6. In Form Body enter the details of the form. Use the formatting toolbar to format the content as required. You can add or edit options to collect information, insert dynamic text, and add attachments.
  7. When you have entered all the form details, select Save.

Add or Edit Form Options

When the information you want to send in your form has been added, form options can be added to collect information from the recipient.

It is important that form options are used as they can be used to produce reports, e.g. how many recipients selected Yes. For more information, go to Form Responses Report.

To add options, position the cursor in the required location, then select the form option required. The following options are available: 

Do not copy and paste to add further form options as the formatting is not applied.

  • [Small Text Box] and [Mandatory Small Text Box] — adds a single line box enabling a small amount of text to be entered on a single line. This is typically used for names, numbers, email addresses, etc.
  • [Long Text Box] and [Mandatory Long Text Box] — adds a multi-line box enabling a larger amount of text to be entered. This could be used for dietary needs or providing additional information that is likely to take up more than one line.
  • [Check Box] — adds a check box for an option to be selected. For example, a yes or no option, or you can list a series of options from which one or more can be selected (such as meal options, uniform sizes, etc).
  • [Option Button] and [Mandatory Option Button] — adds a button for an option to be selected.

Insert or Edit Dynamic Text

You can add dynamic text in the same way that you do for messaging. For example, inserting [STUDENTNAME] is replaced with each student's name when the form is produced.

Position the cursor where the dynamic text is to be inserted, then select the required option from the Insert Dynamic Text A token that is replaced by text when a message is sent. For example, adding [Title], would be replaced with Miss, Mr, Mrs, depending on the title recorded in a database for the recipient. drop-down list. Students who have multiple priority 1 contacts all receive the same form. For this reason, IRIS Reach has removed any dynamic text fields that relate to priority 1 contacts. Any previously set up forms only display the dynamic links and not the information behind it.

Add Attachments

Up to three attachments can be added to a form. For example, if the form is for a trip, you might want to include a PDF with more information or a copy of a group ticket.

Attachments are added by creating Microsites (used to stored one or more pieces of information, which can be accessed via a URL or by scanning a QR code). For more information, go to Create, Edit, and Delete Microsites.

Select the required Microsite from the Pre Made Attachment 1, 2, or 3 drop-down lists. If you want to reference them in the text of the form, use the [LINK1], [LINK2], or [Link 3] dynamic text links.
