CS01 – Confirmation Statement

Introduction

The following document descriebes the features of the confirmation statement form.

 

To access this screen, go to Reports | Company Forms, click Add Event and select Confirmation statement from the Type of entry to log.

The registered company number and the company name in full should be entered followed by the date of the confirmation statement.

The Standard Industrial Classification (SIC) code should be entered in the boxes provided. If a code cannot be determined, the principal activities of the company should be entered in the ‘Principal activity description’ box.

Part 2 of form CS01 requires details of share capital. The class and number of shares, aggregate nominal value and total aggregate amount unpaid, if any in pound sterling and other currencies.

Part 3 of form CS01 requires to include changes to trading status of shares.

Part 4 of form CS01 requires any changes to shareholder details since the information was last delivered to Companies House.

Part 5 of form CS01 requires information about people with significant control (PSC).

 


To activate the form

 

Go to Client | View, select the History tab | Log New. A log client history event screen appears, select Confirmation Statement and then enter the date of the statement. Select Log and it adds the item into the history screen.

To display the form

Once the form has been activated, the completed form can be generated from Reports | Company Forms, highlight form CS01 and select Print Form.

To display a form with basic details, go to Reports | Forms with Basic Details. Select CS01- Confirmation Statement and the form opens in Adobe PDF.

To display a blank form, go to Reports | Blank Forms | Form CS01- Confirmation Statement. This opens the blank form in Adobe PDF.
